Passed away March 24, 2015, aged 54 years.

Will be sadly missed by Family and Friends.

Funeral Service to be held on Monday, April 27, at Gornal Wood Crematorium, at 3.30pm.

Family flowers only but donations, if desired, to British Heart Foundation can be placed in the donation box at the Funeral.
