CHANGE OF NAME



For and on behalf of myself wholly renounce and abandon the use of my former name of MR DIPAK KUMAR OJHA and in place of it do assume from today the name of MR DEEP PATEL and so that i may from today be called and known not by my former name of MR DIPAK KUMAR OJHA but by my new name of MR DEEP PATEL.I expressly authorise and request all persons at all times to address me by my new name of MR DEEP PATEL .
