Of Briarwood, Brookside, Telford, formerly of Bilbrook, passed away peacefully on January 14, 2014, aged 71 years.

Reunited with our beloved Dad.

A dearly loved Mum to Dave, Mandi and Kathy and devoted Nan to all her Grandchildren.

A Service to Celebrate Hazel's life will take place on Friday, January 31, at 2pm, at Telford Crematorium.

Black dress optional.

Family flowers only by request but if desired donations for the RNLI Barmouth would be appreciated.
