Peacefully passed away on Wednesday, October 3, 2012.

Jacquie aged 58 years, extremely loved and fondly remembered Mum of Jody, much loved Daughter of Betty and the late Melbourne and a special Sister of Pauline.

Jacquie will be sadly missed by all her loving Family and Friends.

Funeral Service and Cremation to be held at Bradwell Crematorium, Newcastle-under-Lyme , on Monday, October 15, 2012, at 2pm.

Will Relatives and Friendsplease accept this Intimation and kindly meet at the Crematorium.
