Passed away suddenly at home, on December 13, 2017, aged 46 years.
Beloved Mum to Lewis and Shannon.
Special Nan to Cody.
Much loved Daughter and Sister.
Funeral Service to be held at Telford Crematorium, on Monday,January 22, at 10am.
Family flowers only please but any donations kindly received will be divided between Help for Heroes and SSAFA, where a donation plate will be provided at the Crematorium.
