Passed away peacefully at The Princess Royal Hospital on November 22, 2017, aged 37 years.

Dearly loved and sadly missed by her Son, Jayden. Mum, Hilary. Dad, Dave and Lesley. Brother,Matt, Zoe and Harvey. Brother, Mark, Jane, Isobel and Charlie.

Funeral Service to be held at Telford Crematorium on Monday, December 11, 2017 from 12 noon.

Immediate Family flowers only.

Donations if desired please to the Severn Hospice, Telford, for which purpose a collection box will be provided at the Crematorium.
