Passed away peacefully at the Severn Hospice, in the presence of his Family on November 17, 2017, aged 69 years.

Beloved Husband of Olwen, loving Father of Andrew and John, very dear Father-in-law to Nikki and Anna and a devoted Grandad to Thomas, Evelyn, Erin and Megan.

Much loved, Sam will be sadly missed by all his Family and Friends.

Funeral Service to take place at Shrewsbury Crematorium, on Friday, December 8, at 12.30pm.

Family flowers only please but donations if desired for Hope House or the Severn Hospice.
