Passed away peacefully in hospital on June 26, 2012, aged 67 years.
Will be sadly missed by all the Family.
Funeral Service to be held at, Telford Crematorium, on Wednesday, July 4, 2012, at 11am.
Family flowers only, donations if desired please for The High Dependency Unit at The Princess Royal Hospital, for which purpose a collection box will be provided at the Crematorium
