Passed away peacefully at Portland House Nursing Home, on October 29, 2016, aged 76 years.

Beloved Husband of Claire.

Loving Father of Sharon, Paul and Lynn and a much loved Brother.

Sam will be sadly missed by all his loving Family and many Friends.

Funeral Service to take place at Emstrey Crematorium, Shrewsbury, on Thursday, November 17, at 12.30pm.

Family flowers only please.

Donations if desiredto Portland House Residents Comfort Fund, for which purpose a donation box will be provided at Emstrey.
