Passed away peacefully at the Severn Hospice, Telford on August 30, 2016, aged 68 years.

Jo's Soulmate, devoted Father of Emily, Daniel and Simon and a dearly loved Grandad to Ivy.

Funeral Service to be heldat Telford Crematorium, on Tuesday, September 20, 2016, at 11am.

All floral tributes welcome or donations if desired to the Severn Hospice, Telford, for which purpose a collection box will be provided at the Crematorium.

No black to be worn by requestof Dave.
