Passed away suddenly at The Princess Royal Hospital, on Tuesday, February 10, 2015, aged 47 years.
Loving Father to Shannon and Summer.
Sadly missed by all Family and Friends.
Funeral Service to beheld at Telford Crematorium, on Thursday, March 5, at 1pm.
Family flowers only please.
Donations if desired for The British Heart Foundation, for which purpose a collection box will be provided at the Crematorium.
