Of Oxford Road, Dawley, passed away suddenly on July 27, 2014, aged 45 years.
Dearly loved Husband of Mandy and much loved Son of Gill and Bill.
Funeral Service, 11am, Monday, September 1, at St Michael's Church, Madeley, followedby Interment in Red Lake Cemetery.
Colourful clothing preferred.
Family flowers only by request but if desired donations for Help for Heroes would be appreciated.
