Formerly of Wolverhampton.
Passed away at The Royal Shrewsbury Hospital on June 18, 2014, aged 55 years.
Funeral Service to take place at Telford Crematorium on Thursday, July 3, at 11am.
Colourful dress would be appreciated.
Family flowers only please, donations if desired to The Macmillan Nurses, for which purpose a donation box will be provided at the Crematorium.
