Formerly of Crickheath, passed away suddenly at home in Oswestry, on November 7, 2010.

The Funeral Ceremony will take place on Tuesday, November 23, at Pentrebychan Crematorium, at 12 noon.

Family flowers only are requested, however your very kind donations, in Jack's memory, will support the work of The County Air Ambulance.
