Passed away after a short illness at St Georges Park Care Home, on October 4, 2015, aged 61 years.
Funeral Service, 12 noon, Friday, October 23, at Telford Crematorium.
No flowers by requestbut if desired donations for the Severn Hospice, Telford would be appreciated.
