(Jake) Passed away at The Royal Shrewsbury Hospital on May 3, 2019, aged 28 years.

Jake will be deeply missed by all his loving Family and his Friends.

Funeral Service to be held at the Cemetery Chapel, Longden Road on Thursday, May 23, at 1.30pm, followed by Interment in Shrewsbury General Cemetery.

Family flowers only.
