Passed away peacefully on December 2, 2013.
The Funeral Service is to be held at Streetly Crematorium, on Monday, December 16, 2013, at 10am.
Family flowers only, donations to Help for Heroes.
No matter how life changes, no matter what we do, a special place within our hearts will always be kept of you.
