Passed away peacefully on December 27, 2012, at St Giles' Hospice.
Funeral Service to be held at St John's Church, Heath Hayes, on Thursday, January 10, 2013, at 10.45am and will be followed by Cremation at Streetly Crematorium at 12noon.
Family flowers only, donations for St Giles' Hospice and Matthews Trust Fund may be forwarded to
