Passed away on December 3, 2012, aged 45.

Funeral Service at Codsall Trinity Methodist Church at 12 noon, Thursday, December 20, followed by Committal at Bushbury Crematorium at 1.30pm.

All Friends welcome.

Family flowers only please and facilities at the Church available for donations.
