Passed away suddenly at home, on November 20, 2012, aged 46 years.
Funeral Service to be held at Holy Trinity Church, Bilston, on Monday, December 3, at 1.15pm, followed by Burial at Bilston Cemetery, at 2pm.
All flowers welcome.
