Peacefully passed away on Tuesday, August 14, 2012, aged 56 years, at New Cross Hospital.
Funeral Service to take place at St Mark's Church, Pensnett, on Thursday, September 13, 2012, at 10.45am, followed by Interment at GornalWood Cemetery.
Family flowers only, donations in lieu of flowers to The Heart Foundation please.
