Passed away peacefully on January 19, 2016, aged 44 years.

Funeral Service will be held on Wednesday, February 10, 2016, at Bushbury Crematorium, West Chapel, at 1.30pm.

Family flowers only by request.

A donation box willbe available for Acorns Children's Hospice.

Would anyone attending be kind enough to wear Aston Villa colours.

If flowers grow in Heaven Lord then pick a bunch for us, place them in Dawn's arms and tell her they're from us.Tell her that we love her and when she turns to smile, place a kiss upon her cheek and hold her for awhile.
