Passed away on August 12, 2015, aged 47 years.

Funeral Service to be held on Wednesday, September 2, 2015, at Bushbury Crematorium West Chapel at 3pm.

Flowers welcome or donations if preferred for Autism West Midlands may be placed in the donation box at the Crematorium.
