Who passed away peacefully at Walsall Manor Hospital, on October 13, 2011, aged 68 years.
The Funeral Service is to take place on Tuesday, November 1, at Sandwell Valley Crematorium, West Bromwich, at 1.45pm.
Sadly missed, never forgotten.
