Passed away peacefully at The Princess Royal Hospital, on September 17, 2014, aged 69 years.
Funeral Service at Telford Crematorium, on Thursday, October 9, 2014, at 2pm.
Family flowers only please but donations to be divided between Macmillan Cancer Relief and British Heart Foundation, would be appreciated.
