Hazel sadly passed away at Fernwood Court Nursing Home following a long illness.

Hazel will be sadly missed by all Family members.

Service to be held at St Matthew's Church, Church Hill, Walsall on Monday, August 4, at 2pm, followed by Committal at Streetly Crematorium, West Chapel, at 3pm.
