Passed away on September 13, 2011, at The Manor Hospital, aged 61 years.
Funeral Service to take place on Wednesday, September 21, at the West Chapel, Bushbury Crematorium, at 11am.
Family flowers only, donationsto the running of the Disability / Disabled Over 16 Club.
