Who passed away peacefully at New Cross Hospital on March 7, 2014, aged 73 years.
The Church Service is to take place on Monday, March 31, at St Leonard's Church, Bilston, at 10.15am, followed by Committal at 11.15am in the West Chapel, Bushbury Crematorium.
