Passed away peacefully in Queen Elizabeth Hospital, Birmingham, on December 2, 2010, aged just 50.

We love you David and are filled with happy memories of all the good times we shared, your smile and strength will always be remembered, by so many people, you are an inspiration David and enriched the lives of many and we miss you.

The Funeral Service will take place on Friday, December 10, 2010 at 10.30am at St Luke's Church, Cannock and afterwards to be laid to rest in ActonTrussell Church grounds.
