Passed away September 16, 2010, aged 42 years.
Funeral Service at Gornal Wood Crematorium on Thursday, September 30, 3.30pm.
Family flowers only, donations if desired will be divided equally between Mary Stevens Hospice and Cancer Research UK which will be collected on the day.
No black or dark colours requested.
