Passed away suddenly, at home, on December 14, 2012, aged 38 years.

Funeral Service to be held at Upper Ettingshall Church, Coseley on Friday, January 4, 2013, at 12.15pm, followed by Burial at Beacon Hill Cemetery at 1pm.

Everyone is welcome afterwards at The New Inn, Ward Street, Coseley.

All flowers welcome.
