Passed away peacefully at New Cross Hospital on June 4, 2019, aged 81 years.

Will be sadly missed by Husband Frank, Children Neil, Alan and Lynda, Grandchildren and Great-Grandchild.

Funeral Service will be held at Bushbury Crematorium, West Chapel on Monday, June 17 at 12 noon, followed by Interment at Pinfold Lane Cemetery, Penkridge.

Donations in memory of Hazel will be passed to Cancer Research UK.
