Passed away peacefully on Saturday, March 28, 2015, at University Hospital, Stoke-on-Trent, aged 36 years.

Dearly loved Son of Alan.

He will be sadly missed by all his Family and Friends.

Funeral Service will take place at Bushbury Crematorium West Chapel on Thursday, April 23, at 1.30pm.
