Sadly passed away at home, on Sunday, November 18, 2012, aged 57 years.

Funeral Service to be held at Bushbury Crematorium, West Chapel, on Friday, November 30, at 11.30am.

Family flowers only. Donations if desired to Macmillan Nurses.

Everyone is then invited to join the Family at Penn Bowling Club, Manor Road, Penn.
